Wilkinsburg Tree Request Form

Please read this form and fill out the appropriate fields below if you are interested in getting a tree!

This is a request for the Borough of Wilkinsburg to plant tree(s) on the public right-of-way adjacent to my property. I understand the planting of trees may necessitate the Borough of Wilkinsburg cutting an opening in the sidewalk approximately 3-feet wide by 10-feet long to accommodate the tree planting.

I hereby acknowledge that I am the owner of the above said property. I hereby request the Borough of Wilkinsburg to supply and install a tree(s) to be located on the municipal right-of-way adjacent to my property. I understand that there is no charge for this service. I also understand this form does not guarantee that a tree(s) will be placed adjacent to my property.

Furthermore, I understand that street trees and trees in the public right-of-way are the property and responsibility of the Borough of Wilkinsburg. Trees planted outside of the public right-of-way are the responsibility of the property owner. Only the Borough of Wilkinsburg can authorize major pruning or removal of street trees.

I also understand this form does not guarantee that a tree(s) will be placed adjacent to my property. Upon receipt of this request, a TreeVitalize representative will do a site analysis to determine the suitability of planting a tree at this location.

Required Conditions

I understand my site may require removing concrete, sidewalk or paving materials in order to plant this tree, and I give permission to have this done.

I understand that the species of tree will be determined by a Tree Vitalize representative.

I am willing to share in the responsibilty to care for the authorized tree(s).

I am willing to help care for other trees in my neighborhood.
